Employee Action Report Help

The following instructions have been developed to assist you in using the Employee Action Report (EAR) system.

Setting up an Account
  • To set up an account with the Division you must Register

Signing On
  • Enter your account number and password.
  • If you would like to change your password, enter the new password in the new password field and in the verify new password field.

Selecting an agency license.
  • After you have successfully signed on, the Employee Action Report menu will automatically be displayed.
  • Select the agency license you will be using for this session.
  • Select the desired employee action Hire, Terminate, or Change Address.

Hire an employee.
  • Enter the employee’s license number and social security number.
  • Once you have entered a license number and social security number, the employee data will be retrieved. Verify that the information is correct.
  • Enter the hire date in MM/DD/YYYY format.
  • Submit your request, press Confirm.
  • If no errors are encountered, a message will be displayed to indicate that your transaction was successful.
  • You can choose to perform another EAR action by using the EAR navigation menu.

Terminate an employee.
  • Enter the employee’s license number and social security number to be terminated.
  • Once you have entered a license number and social security number, the employee data will be retrieved. Verify that the information is correct.
  • Enter the termination date in MM/DD/YYYY format.
  • Enter resignation indication (Y=Yes or N=No), default is N=No.
  • If the termination was due to a 493 violation, check the appropriate cause for dismissal.
  • Submit your request, press Confirm.
  • If no errors are encountered, a message will be displayed to indicate that your transaction was successful.
  • You can choose to perform another EAR action by using the EAR navigation menu.

Change an employee's address.
  • Enter the employee’s license number and social security number.
  • Once you have entered a license number and social security number, the employee data will be retrieved. Verify that the information is correct.
  • Enter the zip code for the new residence and/or mailing address.
  • Submit your request, press Select Zip Code.
  • A list of cities that contain the zip code(s) entered will be displayed along with blank fields to enter new address information.
  • Enter the new address information.
  • Select the desired city from the drop down list provided.
  • Submit your request, press Confirm.
  • If no errors are encountered, a message will be displayed to indicate the address change was successful.
  • You can choose to perform another EAR action by using the EAR navigation menu.

View current employees (list).
  • From the EAR navigation menu, select View Employee List.
  • A listing of the agency's current employees, up to 50 at a time, will be displayed.
  • If the agency has more than 50 employees, select Next List.
  • You can choose to perform another EAR action by using the EAR navigation menu.

How to use EAR navigation menu.

Once you have successfully signed on to EAR, you will see the Employee Action Report (EAR) navigation menu, located on the left hand side of the page.

Like the other navigation menus found on the Division's web site, you can use the EAR navigation menu to select employee action options at any point within your EAR session.

Note: The navigation options will change during your session, depending on the data requirements for a given action.

What is the format of the license number?

The format of the license number is the license prefix and a 7 digit number. If the license prefix consists of one letter, then the prefix would be that letter and a space.

Example: The format of an "A" license would be "A 1234567". The format of an "AA" license would be "AA1234567".

What does an agency do when a new employee has not yet been assigned a license number?

An agency can update an employee's record only if the employee has a valid license number or an application on file.

If the employee you are hiring has recently submitted an application to the Division of Licensing, wait a few weeks before you add them to the system using the Employee Action Report.

If you cannot find the license number using the Individual Search after two weeks, complete the Employee Action Report form, DACS-16006, and mail it to the Division of Licensing for further research to be performed.
